Cleveland Heights 4-Ball

I understand and affirm that the Cleveland Heights 4-Ball (herein after known as 4-Ball) and course owners and/or operators are not liable for any accidents or injuries suffered while participating in 4-Ball sponsored events. By completing the registration process, I agree to fully indemnify and hold harmless the 4-Ball and its agents, representatives and volunteers from any damages suffered by me, my family or my child while participating in, or traveling to or from any program sponsored by the 4-Ball. I authorize the 4-Ball to release my scores and photographs to any newspaper, golf publication, golf website, golf organization. I have read and agree to the terms set forth.

Your entry fee will include a tee gift, BBQ and free drinks on the course Saturday and Sunday during the tournament, dinner and band Saturday night and a breakfast or lunch buffet both days depending on your tee time.

PLEASE NOTE, the 4Ball is an invitational golf tournament for amateur players only.  If you did not receive your invitation directly from our Board President, Tim Darby, and you are a new participant attempting to register, your payment will be accepted, however, your team will be placed on a waiting list and will be added to the field if spots are available based on the flighting needs of the tournament field. Completed registration and payment DOES NOT GUARANTEE ENTRY INTO THE EVENT. 

Tournament Rounds will take place on Saturday April 4^th and Sunday April 5^th.
